Cash Request Form

Please use this Cash request form to request funds. to request initial funds (90% of award) from Florida Humanities for your Book Festival Grant. Please note that the final 10% of the award is to be asked for in a cash request found in your Final Report follow-up, located in the Online Application Portal.

Please use this form to formally submit your proposed grant change, including all programmatic and/or budget changes. All proposed grant changes must be sent to Florida Humanities at least two weeks prior to change for approval.

Acknowledgment

Awarded funds are provided by Florida Humanities through a grant from the National Endowment for the Humanities. All materials publicizing or resulting from award activities must contain an acknowledgment of Florida Humanities and the National Endowment for the Humanities support and carry the appropriate logo.

The acknowledgment must also include the following statement:

“Funding is provided in part through a Florida Humanities Book Festival Grant with funds from the National Endowment for the Humanities and in partnership with (YOUR ORGANIZATION NAME). Any views, findings, conclusions or recommendations expressed in this program do not necessarily represent those of Florida Humanities or the National Endowment for the Humanities.”

Acknowledgement

The following exhibition credit line and logos for the Smithsonian Institution, Florida Humanities, and the National Endowment for the Humanities must appear on all exhibition-related promotional materials. This statement does not need to be in the same size font as that of the sponsor logos, nor must it be located immediately adjacent to the logos. Type in the logos must be legible and no smaller than 5 points. Please consult your Museum on Main Street Local Coordinator Handbook for further details.
